Getting discharge right: making change happen in hospital discharge and intermediate care

Hospital discharge continues to be a key challenge facing health and care systems. The PSC is delighted to be hosting a roundtable to hear how leaders in UK health systems are driving positive changes in discharge & intermediate care. We will be joined by a panel of national experts:

Why discharge and intermediate care?
National data shows that every day in England, around 15,000 people are in hospital that shouldn’t be. Delays are harmful to patients, difficult for families and they increase pressure on hospitals – driving longer ED wait times, slower ambulance handovers and more patients waiting for planned care. They are also costly – the National Tariff Workbook (2023) estimates the cost of excess bed days at £349.
The PSC Transformation Team are working in health and care systems across the country, diagnosing issues behind discharge delays and piloting interventions in acute and community settings. We are eager to share our learnings from this work and create a space for health and care leaders to share best practice and thought leadership in this area.
